Who needs to download and complete this form? Adults
If this is your first West Virginia mission trip OR if your last background check was prior to 2006 (over 3 yrs old) we need you to download this background check form and submit it to the HBA office (HBA, PO BOX 1104, Waynesboro, GA 30830) or your VBS Director.
